To join or renew by postal mail with a check, first you will need to print and complete our simple application form:

- 2012 Membership Application Form (Adobe PDF)
Next, please mail your completed application form to us at the following address, along with an enclosed check made out to “Fountain City Business & Professional Association” for the $45.00 application fee.
